Our team of hospice professionals works together with the patient and family to customize the patient’s care plan. The hospice process begins when a patient’s personal physician certifies they are eligible for hospice care. If the patient has no personal physician, you can request hospice services by calling Devotion Hospice for guidance and assistance. Our admissions staff is available to evaluate hospice eligibility and answer questions at any time.
